Performance Vector Unmanned Enrollment Form

After this form is submitted USAIG will send confirmation to the policyholder’s designated contact along with an activation code to enable registering or redeeming the allocated SafetyPoints. Each UAS policy is eligible for one enrollment per policy year. Re-enrollment is not automatic after policy renewal and must be requested by the policyholder and activated annually by submission of this form.
