When You Experience a Loss

If an accident or other loss occurs, time becomes critical and accurate reporting essential. We’ve provided convenient forms to help ensure that you gather and report the information we need to process your claim and provide quick, effective assistance.

Keep in mind that your insurance coverage may or may not include all types of accidents or damage mentioned here or on the forms. Nothing on this website, or on the forms, alters the coverages available to you under your USAIG policy.

Aircraft Accident Form

Please use this form whenever an insured aircraft is involved in any fashion, and injuries to persons other than the pilot, or damage to property other than the insured aircraft may have occurred.

Other Than Aircraft Accident Form

Please use this form for all other accidents or incidents which may have caused damage to someone else’s property or bodily injury to any person other than the occupants of the insured aircraft. These would include such items as “slip and fall” events, “hangar rash” to other than the insured aircraft (unless caused by the insured aircraft), claims for incorrect or inadequate maintenance to someone else’s aircraft, pedestrian accidents on the insured’s ramp, painting overspray damage and so on.

Hull Loss Form

Please use this form when there are no injuries to anyone other than the pilot, and no damage to anything but the insured aircraft.
